Patient Registration for Appointments 

Please complete all steps below

Step 1: Complete and submit both forms below to register for the appointment, at least 10 days before the appointment. Both are required.

Step 2: Specialist copayment for Telehealth visit/ Office Visits/ Allergy injection visits should be made prior to the visit as per your insurance company requirements. All co-payments and any balance are due at the time of service 

Visit

Optional Telemedicine visits are available for all patients

Telemedicine is the practice of medicine using technology to deliver care at a distance, as per your insurance company specifications.

Dr. Krishna is able to see and talk with you using a computer in a safe and secure manner. At the telemedicine visit, the doctor can prescribe medication to improve your symptoms. This Telemedicine visit will help determine appropriate testing, if needed, to be authorized by your insurance company as well as prepare ahead for your upcoming visit to our office. The telemedicine visit may be followed later by necessary procedures as appropriate for your condition to identify your allergic triggers and lung function assessment with an on-site visit with the allergist in a few weeks following the telemedicine visit.

If your telemedicine visit is during busy office hours, the video call may be delayed due to in-office patient care. If this happens, please call us back!

If you are experiencing symptoms that could prevent you from entering our office, such as a cough, or have had possible exposure to a potential positive COVID-19 individual, please reschedule your on-site visit.

Telemedicine Instructions

After you receive the video call invite from your doctor, you will be asked to enable your camera and microphone on a screen that looks like the one above. Tap ‘Continue’  or 'Allow'. If your phone is set up to automatically deny permissions you must change the settings to allow Camera and Microphone access. 

In order to join your video call, you must grant your smartphone browser access to your camera and microphone.

Referral

We participate with most major insurance companies and managed care plans (please see list of accepted insurances under Visit - Insurance info tab on this website. All HMO and managed care plans require a referral for services. It is your responsibility to obtain any necessary referrals (minimum 3 visits). Please contact your primary care physician for the referral and bring the it with you to the initial office visit.

Payments

Your balance due amounts are determined by your insurance provider. For the latest statement, please check your email or you can contact your insurance company for your cost share details for services performed at out office.
